Licensed Funeral Director
Lori Diaz is a Licensed Funeral Director and Limited Lines Insurance Agent in the State of Ohio. Her passion for people motivated her to choose this career path and it’s very evident in the care and manner in which she has an affinity for families. She opened Affinity Memorial Chapel in December of 2017. She is proud to serve the families in Central Ohio and considers it a privilege to serve well.
An avid learner, Lori’s educational career path includes a B.A. in Communication from The Ohio State University, Master in Business Administration and a Master in Project Management from Keller Graduate School of Management at DeVry University. Additional interests and proficiency are in marketing and public relations.
Lori is an active and committed member of the Ephesus Seventh-day Adventist Church in addition to other civic, social and professional organizations she has belonged to over the years: National Funeral Directors Association (NFDA), Ohio Funeral Directors Association; National Funeral Directors and Morticians Association (NFDM&A) Buckeye State Funeral Director and Embalmer Association (BSFDEA); African-American Funeral Professionals of Central Ohio (AAFPCO) (co-founder); National Coalition of 100 Black Women Central Ohio Chapter (charter member); and her beloved Zeta Phi Beta Sorority, Inc., Gamma Zeta Zeta Chapter. After one year of membership, her dedication and zeal for service and community yielded the honor of being named the State of Ohio Zeta of the Year in October 2016. She currently serves as the Co-Chair of Z-HOPE (Zetas Helping Other People) which is the community service component of the chapter. She serves as a member on several additional chapter committees.
In her leisure, she cherishes time spent with her loved ones, reading, roller skating, line dancing, traveling and navigating social media. For over 21 years, Lori has been married, blessed to be called mother by their two adult daughters . She recently after 18 1/2 years had their beloved family pet named, Hershey to pass. (His obituary is listed on our site). She realized in that encounter that every life is worth celebrating even our pets.

Pastor Charles Dawson, born and raised in Columbus, Ohio. He grew up just down the street from Affinity Memorial Chapel in Lincoln Park Apartments and is a proud graduate of Brookhaven Senior High School.
Pastor Charles holds an Associate Degree from Columbus State in Micro-Computing Technology, a Bachelors Degree in Substance Abuse Counseling from Ohio Christian University and a Masters Degree in Health and Human Wellness from Liberty University.
Pastor Charles is a United States Army Veteran and Retiree from the Veterans Administration and is currently active with several veteran groups to help those who suffer with PTSD. Pastor Charles serves as Chaplain for Walleye for Wounded Heroes.
He is the Founder of Restoration & Healing International Ministries along with his wife, Michelle Dawson who is Co-Pastor and Co-Founder of the Ministry together they have 6 children and 8 grandchildren. He is a true worshipper who loves the Lord and loves giving back to his community.
Patricia Mullins born in Williamson, WV but moved to Columbus with her family at an early age. Patricia is a graduate of West High School and Bliss Technical School.
Patricia worked for Columbus Police Department in various departments. She retired after 38 years of dedicated service. Patricia is currently a member of Faith Ministries, where she is a member of the Media Ministry, One Accord Choir and Member Services.

Calvin is Affinity Memorial Chapel’s first Client Care Consultant. He joined Affinity in the beginning of December 2017.
After graduating from FCI Academy in June 2008, Calvin began personal assisting varies clients throughout the Ohio Region. In the year of 2016, Calvin developed a passion to assist and help families who have lost a loved one. Calvin is passionate and desires to provide excellence to the families who are in need during their time of loss.
Calvin is planning on continuing his education here in the Columbus, Ohio area with hopes to being a Funeral Director in the near future. Calvin is a native of Columbus, Ohio where he currently resides.
Grief Management Consultant
As the mother of an autistic child and having lost a husband and a son in auto accidents, certified Grief Management Specialist Ruth-Ann Thompson provides guidance for enduring life’s storms. Drawing from her personal experiences she has armed groups across the country with emotional and other tools for effectively navigating life’s hardest challenges.
Ruth-Ann J. Thompson is an author, songwriter, motivational speaker and singer. To date she has written over 70 songs, including praise and worship songs used nationally. In 2012 her first CD, In the Garden, was released. A Thin Place was released four years later, after the death of her son. She authors the children’s series, The Open Fielders.Together, she and her husband, Pastor Kirk Thompson, have six children and four grandchildren.
In her spare time she enjoys reading, sewing and music. Her favorite verse is Job 23:10. “But He knows the way that I take; when He has tested me, I will come forth as gold.” She secretly desires to be a race car driver.
Marcus Mewborn was born in Norfolk, VA, and was raised in Columbus, Ohio. Marcus is the 2nd son of Mary Mewborn-Broadus and the late Leroy Mewborn. He graduated from Walnut Ridge in 2002.
After graduating high school he later graduated with an Associate’s degree from Freedom Bible College & Seminary (2004). Biblical Studies was his major and in addition to that he also received a Certificate of Ministry in Biblical Studies from the Full Gospel Baptist Christian Bible College
In 2004. Marcus has had an active resume’. Some of his career opportunities has included him working as a mail carrier for the U.S. Post Office (4 years) and then he transitioned over to Central Ohio Transit Authority (COTA), where he has been a driver for 7 years. In addition to operating the buses he’s also a union steward with the union that represents the driver’s, Transport Workers Union Local 208.
In his spare time he enjoys: traveling, bowling, and spending time with his family & friend’s.
Marcus inherited a lot of wisdom from his late maternal grandmother who told him, “to never give up on the Lord as he will never give up on you”.

Delvonta “DJ” Davis was born in Columbus Ohio, where he was raised all of his life. He graduated from FCI Academy and at an early age DJ was interested in Health Care, however upon becoming an adult that interest changed from Health Care to Mortuary Science.
Dj is a loving husband and caring father and he loves doing everything he can to make families happy and get through the difficult time of loosing a loved one. Outside of his passion for the industry and being a great servant at Affinity Memorial Chapel, he also serves in ministry. He is the founder and Ministry leader of Judah Break Free Movement, (JBM) a young adult singing group that he started in his late teen years. DJ is spiritually rooted in Christ and he puts him above everything he does.
